Disability Connections (DC) is a non-residential facility available to teach individuals with disabilities how to live a better life with dignity, respect and as a valued member of the community. Services include informational resource referral, one-on-one peer support, systems advocacy, independent living skills training, transition, home accessibility modifications, assistive technology demonstration and loan, and durable medical equipment recycling.
DC serves the following counties: Baldwin, Bibb, Crawford, Houston, Jasper, Jones, Monroe, Peach, Pulaski, Putnam, Twiggs and Wilkinson.
EyeCare America is a national public service program of the Foundation of the American Academy of Ophthalmology. It offers eye care to those who qualify. In order to receive a referral, a client must go on the EyeCare America website to fill out a questionnaire.

Eye Exams & Eyeglasses
The Georgia Lions Lighthouse Foundation has a mobile clinic that visits 26 sites through Georgia (Closest locations listed below). At each clinic, qualifying individuals can receive a fully dilated eye exam and one pair of eyeglasses, which are produced at the GLLF Chamblee clinic. The clinics allow the Lighthouse and its partners to diagnose and treat serious eye conditions before the client experiences permanent vision loss.
Patients can also receive follow-up care for chronic eye conditions, such as glaucoma and diabetic retinopathy. Visit the website to apply. Once an applicant has sent in their information, they will be contacted regarding their acceptance or denial in the program.

- Diabetes related vision loss

- Meet financial guidelines
InfantSEE, developed by the American Optometric Association and The Vision Care Institute of Johnson & Johnson Vision Care Inc., is a public health program designed to ensure that eye and vision care becomes an integral part of infant wellness care to improve a child’s quality of life. InfantSEE feels so strongly about the importance of healthy vision that participating member optometrists will provide a no-cost comprehensive infant eye and vision assessment within the first year or life.

New Eyes For The Needy
New Eyes For The Needy purchases new prescription eyeglasses for U.S. residents. Individuals cannot apply for the program themselves. The online application is only accessible to social service agencies who apply on behalf of their clients.
Prevent Blindness Georgia
Prevent Blindness Georgia provides retinal screenings for adults at risk for glaucoma and other aging eye diseases, free eye exams, and free or low-cost glasses in many areas of the state.
VISION USA was established in 1991 to provide basic eye examinations to Americans in need. If you need an eye exam and do not have insurance and cannot afford one, work with your social worker, case worker, charitable organization or agency to submit an application on your behalf. Applications can be found online and MUST be submitted by a referral agency.
